What is Old Town Family Medical Clinic?
Old Town Family Medical Clinic is an independent membership based primary care practice built around time, access, and the physician-patient relationship. Rather than focusing on volume, we care for a smaller number of patients so we can provide more personalized, thoughtful medical care.
Who is Dr. Jonathan Bloch?
Dr. Bloch is a board-certified family physician with more than 25 years of experience. Before attending medical school at Des Moines University, he earned my undergraduate degree from the University of Colorado, where he was the university's first graduate in Behavioral Neuroscience and Psychobiology,
His internship and residency training took him to several parts of the country, from Florida to northern Idaho, and he’s practiced in a variety of settings including family medicine, urgent care, occupational medicine, emergency response, and direct primary care. he moved to Humboldt County in 2018 to escape city-life and has not looked back.
Do you accept insurance?
Yes. We accept Medicare and several major commercial insurance plans. The practice combines traditional insurance with a membership. The membership fee works alongside your insurance, it doesn't replace it. Insurance is billed, and copays are still collected for covered exam services. The membership covers non-reimbursable services, expanded communications with your physician, and allows for a more personalized style of primary care that is extends beyond what any insurance company will reimburse.

What is it like to be a patient?
Old Town Family Medical Clinic functions like any modern primary care practice. Dr. Bloch diagnoses and treats acute and chronic medical conditions, prescribes medications, orders laboratory testing and diagnostic imaging, coordinates specialist referrals, and manages overall healthcare just as any family physician.
Dr. Bloch is also available for urgent evaluations and care, including sick visits, injuries, IV therapy, laceration repair, and splinting.
When you need blood work, X-rays, CT scans, MRI studies, ultrasounds, or other diagnostic tests, we order them. When specialty care is appropriate, we coordinate your referral and remain involved as your primary physician. We also review outside records, hospital reports, laboratory results, and consultant recommendations so your care stays coordinated.

A comprehensive annual physical examination per year and unlimited follow-up examinations.

Discount pricing for vitamins and nutritional supplements from Pure Encapsulations®.

Access to the Old Town Sanctuary, designated outdoor wellness spaces located at The Gregor Nook & Gable.
How much is membership?
Charter memberships for adults are currently $1,800 per year ($150/month). This introductory rate is available to a limited number of founding patients who join while the practice is being established.
Children of enrolled members through age 26 are $600/year ($50/month).
Optional Osteopathic Manipulative Treatment (OMT) is available for an additional $1,200/year ($100/month) for patients who want to benefit from regularly scheduled adjustments.
Can I use my HSA, FSA or other tax advantaged funds?
Yes. Membership fees are designed to qualify for federally accepted tax advantaged funds. Patients should verify eligibility with their plan administrator or tax advisor.
Do I have to commit long-term?
No. Memberships may be paid monthly, quarterly or annually, and can be canceled at any time without penalty. We want patients to stay because they value the service, not because they feel obligated.
What if I have complicated medical problems?
Dr. Bloch enjoys caring for patients with complex medical conditions. In fact, this is where a real life physician will make the greatest difference.
Modern medicine has incredible specialists, tests, and treatments. But when symptoms don't fit neatly into a diagnosis, or when someone has several conditions at once, what's often needed most is an experienced physician who can step back, listen carefully, think critically, and put all the pieces together.
Establishing the correct diagnosis is one of the most important things a physician does. It requires time, experience, careful observation, and sometimes the willingness to question previous assumptions.
Dr. Bloch may not always have an immediate answer. Some conditions and complex disorders may take time to understand. The priority is to keep looking carefully, make sure we aren’t missing anything, treat what we can, and help you achieve the best possible function and quality of life.
Sometimes the most valuable thing a physician can offer isn't another test or prescription. It's careful thinking, honest communication, and a commitment to working through difficult problems together.
